Please assist us by providing the following information at the time of your appointment:

• Any X-rays if applicable
  
• A list of medications you are presently taking
  
• If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  

Generally your first appointment will include an exam from one of our dentists, and a cleaning and necessary x-rays with one of our registered dental hygienists. If there is a specific concern or if emergency treatment is necessary you will be appointed with one of our dentists for an examination or consultation explaining your diagnosis and treatment options.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the consultation visit.
