For your convenience we accept Visa, MasterCard, Discover and American Express. We deliver the finest care at the most reasonable cost to our patients. Payment is due at the time service is rendered unless other arrangements have been made in advance.

Insurance Information
Please bring your insurance information with you to your first visit so that we can expedite reimbursement. We are providers for Delta Dental and Blue Cross Blue Shield but will gladly file to your insurance company or assist you with you claim. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We have teamed up with Citi Card and Capital One to provide several options on payments for your convenience, visit www.healthcard.citicards.com or www.dentalfeeplan.com.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
